Initial Exam/First Visit

On your first visit, our objective is to determine the long-term needs for you and/or your child. At this visit, one of three outcomes is determined:

  • Treatment is needed now.
  • Treatment is not needed.
  • Treatment is needed, but the timing is such that it should be delayed until appropriate growth, tooth eruption or other factors are monitored and reconciled. This process is Observation Recall.

Dr. Campbell does not compromise his time commitment to your exams, and spends very valuable time with you delving into the inadequacies of your bite (occlusion), your smile arc and width, your dento-facial relationship to include profile concerns, and any other concerns you may have. In certain cases, records will need to be taken and a follow-up, pre-treatment consultation scheduled to discuss treatment options. ;Any records taken in order to complete a diagnosis and finalize treatment plan options are at cost.

Please have the appropriate paperwork completed when presenting for your initial exam. Our Patient History forms are available at the bottom of this page. You can also ask us to send you the form upon contacting us! Please also bring your insurance card(s) if you have orthodontic benefits; this is a sub-benefit of dental benefits, so just having dental benefits does not ensure you have orthodontic benefits.

If Dr. Campbell feels any records are necessary in order to give a proper diagnosis, he will ask his Patient Coordinator to take these at the beginning of the exam. He then will present his findings and recommended treatment plan while displaying these records on a projection screen. Because orthodontics is a substantial investment, we understand that you may not be willing to begin the treatment process or agree to treatment the day of the exam. Don't worry, we give you a detailed letter with our findings and recommended treatment plan in lay terms and any other applicable materials to take home and consider.

If no follow-up consultation is necessary, the Patient Coordinator will present you with the total treatment fee and payment options at the initial exam appointment.

If you have Temperomandibular Dysfunction (TMD), or jaw joint, symptoms, please complete and electronically submit or print and bring this form.
